We are excited to offer an exceptional opportunity to join the newly established Neurodegenerative Team within the South Western Sydney Local Health District.As a Social Worker (Level 3), you will play a vital role in providing holistic care to individuals across the district facing neurodegenerative conditions, with a primary focus on Motor Neurone Disease.Collaborating closely with a multidisciplinary team, you will lead the development of district-wide pathways to improve access to specialised care for our valued clients.By forging strong partnerships with departments such as Neurology, Palliative Care, and Respiratory Medicine, you will contribute significantly to enhancing the quality of life for those affected by neurodegenerative diseases.Join us in making a meaningful impact and advancing transformative care delivery in SWSLHD.What You’ll be Doing
This position is responsible for providing specialist Social Work service to consumers and their carers particularly in the Neurodegenerative Disease Supportive Care Service to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct.The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.Where You’ll Be Working

To be considered for this position, please ensure you address the selection criteria as thoroughly as possible. * A Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
- Extensive post graduate experience working as a Social Worker in relevant clinical areas in a health setting, including a demonstrated understanding of complex health conditions and the impact on wellbeing.
- Demonstrated experience in prioritise work and manage stressful situations; and apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature.
- Demonstrated experience in the planning, implementing, leading and evaluating of quality improvement activities involving the application of evidenced based practice within social work
- Demonstrated ability to lead complex casework including communication and collaboration with the patient and other stakeholders in order to maximize positive patient outcomes and collaborative partnerships
- Demonstrated counselling skills to address client-distress, and ability to provide counselling / therapies that focus on the psychological, behaviour, relational or social causes of distress
- Experience working independently and collaboratively within a multi-disciplinary team.
- Demonstrated high level communication skills (written and verbal), computer and prioritisation skills
- Demonstrated experience in the provision of clinical supervision and expert advice/consultation to the social work service and the clinical specialty.
- Must possess a valid Australian Drivers Licence (excluding learner driver’s licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
