Essential Functions:

  • Serve as the major contact person for new residents at move-in and help with residents’ orientation and transition.
  • Assess the psychosocial status of residents related to their physical status and current environment.
  • Develop one-on-one relationships with residents and families as needed for counseling.
  • Document all interactions with residents and/or family in the assessment and care plan as required by state standards. Participate in Family Meetings as needed.
  • Act as a liaison with social, health and community agencies.
  • Assist with special events as needed.
  • Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
  • In cooperation with community life, plan, schedule, and implement room visits and in-room activities for residents unable to leave their rooms.
  • Establish support groups to provide residents and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses for The Inn and The Gardens.
  • Responsible for submission of information for proper billing of tiered level services for The Inn.

What you will need for this position:

  • Masters of Social Work degree preferred; experienced in human services, social work, or a related subject required; geriatric experience.
  • 2-4 years experience in the field of human services or social work providing services to an adult population; preferably in Director level capacity in an Assisted Living community. Knowledgeable of state regulations.
  • Excellent leadership and communication skills.
  • Demonstrates an understanding of patho-social changes that is associated with the aging process and disabilities that is used to develop interventions.
  • An equivalent combination of experience, education, and other qualifications may be considered.
