What you’ll be doing

The Aboriginal Registered Nurse is responsible for the provision of high quality, professional nursing care that is underpinned by compassion and consistent with NSW Health and ISLHD values, policies and standards.

The Aboriginal Registered Nurse is required to work to their scope of practice. The Aboriginal Registered Nurse is to retain responsibility and accountability for their personal and professional actions.

The Aboriginal Registered Nurse will work with the Enrolled Nurse and Assistant in Nursing, and ensure that delegated functions are suitable to their scope of practice.

The Aboriginal Registered Nurse is responsible for the provision of person centred; evidence- based nursing care that promotes positive health outcomes and foster the Chronic Care for Aboriginal People (CCAP) Model of Care framework to underpin a shared care approach to manage the care of Aboriginal people with a chronic disease. The Registered Nurse will work:

  • Within a multidisciplinary team within Virtual enhanced Community Care (VeCC)/Connecting Care service (CC) that is co-located with Access and Referral Centre (ARC) and Northern Community Nursing Team (NCNT) to support community based chronic disease services for Aboriginal clients across the District by facilitating linkages within and between ISLHD, general practitioners (GPs), non-government organisations and Aboriginal Community Controlled Health Service.
  • To support the Aboriginal Chronic Care Unit (ACCU) Connecting to Country (CTC) team consisting of Aboriginal Health Workers (AHWs) at both Port Kembla Hospital (PKH) and Nowra Community Health Centre (NCHC), as required.
  • To improve the transition of care from hospital to community care by ensuring processes are formalised between Aboriginal specific services across the care continuum, which includes guidance regarding the integration of CTC support with VeCC/CC service delivery.

This position is open to Aboriginal and Torres Strait Islander applicants only in accordance with Clause 26 of the Government Sector Employment Rules 2014, applicants must demonstrate Aboriginality in addition to addressing the selection criterion.

All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

Selection Criteria * Must be of Aboriginal and/or Torres Strait Islander decent. An applicant’s race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW.

  • Current registration with the Australia Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse 2nd Year or above and a current NSW drivers licence class C and willingness to travel according to the job demands of the role.
  • Demonstrated ability to deliver person-centred care in the relevant health care setting.
  • Demonstrated ability to effectively lead and work in an interdisciplinary team.
  • Demonstrated ability to manage competing priorities in a time efficient manner.
  • Demonstrated ability to use evidence to inform clinical practice.
  • Demonstrated experience and clinical skills in chronic disease nursing practice
  • Demonstrated experience in working with Aboriginal and Torres Strait Islander people and the ability to provide effective and culturally appropriate guidance/ education to staff, clients, carers and community members.
