(LBVC) is a state-wide program that is accelerating in NSW. It involves clinicians, networks and organisations working together on high-impact initiatives to improve outcomes and experiences for people with specific conditions. Changing care delivery and the patient experience from “what’s the matter with me” to “what matters to me”.
This role will lead the implementation, monitoring and evaluation of LBVC initiatives across the Nepean Blue Mountains Local Health District. This requires continually striving to design and deliver care that delivers the four essentials of value:
- health outcomes that matter to patients
- experiences of receiving care
- experiences of providing care
- effectiveness and efficiency of care
A key priority for the District is the embedding and scaling of initiatives and improving pathways for care in the community and outpatients to fully realise benefits through sustained change. And in turn raising awareness and understanding of the benefits of value based healthcare. LBVC is a collaboration between the (MOH), (ACI), (CEC) and and local health districts and networks. Job share will be considered.
An eligibility list may be created for future vacancies.
