The position plays a lead role in facilitating relationships between Aboriginal patients and Health professionals, advocating for the needs of Aboriginal patients by educating and advising Health professionals on the delivery of culturally safe and secure healthcare.The PAHW will develop models of care for Aboriginal people to ensure a safe and culturally appropriate transition of care between the primary prevention, primary care, hospital based care and highly specialised tertiary health services, with improved discharge and onward referral processes. The PAHW and Aboriginal Health Workers (AHWs) play a key role in combating the high burden of disease and mortality rates in Aboriginal communities and works within a primary health care framework to achieve better health outcomes and better access to health services for Aboriginal people.The PAHW may provide clinical supervision and cultural support for other AHWs across Illawarra Shoalhaven Local Health District (ISLHD) hospital and community based settings.The position works collaboratively with the Social Work Departments and multidisciplinary teams within hospital and health services, and with patients shared across the Illawarra Shoalhaven Local Health District.   • Must be of Aboriginal and/or Torres Strait Islander descent and have tertiary (degree) qualifications in a field related to Aboriginal Health.
  • Demonstrated advanced skills in Aboriginal primary health care ability including casework and counselling and demonstrated experience and ability in supervising and providing leadership to Aboriginal Health workers.
  • Demonstrated ability to engage and work effectively with Aboriginal and Torres Strait Islander communities including the local Aboriginal community. Knowledge and understanding of Aboriginal and Torres Strait islander cultures, health and welfare issues/services and needs of Aboriginal and Torres Strait Islander people.
  • Demonstrated ability to work independently, show advocacy skills, and ability to prioritise workloads and provide clinical supervision and cultural support.
  • Demonstrated ability to work effectively as part of a multidisciplinary team and develop partnerships and relationships across health service teams.
  • Well-developed written and verbal communication skills and computer literacy
  • Knowledge of quality improvement and demonstrated ability in development, implementation and evaluation of health promotion and health education programs, and the capacity to implement strategic directions of the NSW Aboriginal Health Plan 2013-2023.
  • Current NSW driver’s licence.
