Compensation: In accordance with California law, the expected salary range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.About the Program:
- Transitional Housing Program: This program provides housing and supportive services to non-minor dependent pregnant and/or parenting young women ages 18-21 who are still in the foster care system, as well as pregnant and/or parenting young women ages 18-24 who were or have transitioned out of the foster care or juvenile justice system.
Role Summary: This position will be under the general direction of the Director of Supportive Housing and will perform intake coordinator responsibilities and linkage to stable housing. The Housing Access & Support Coordinator will also provide focused services to Supportive Housing aftercare clients. Duties and responsibilities include but are not limited to: Identifying clients in need of services, providing client advocacy and case management for current and alumni clients.
- Schedule: Monday through Friday, 9:00am to 6:00pm, with every other Friday off 9:00am to 5:00pm (9/80 schedule)
- Location: 155 N. Occidental Blvd Los Angeles, CA 90026
Duties and Responsibilities:
- Conduct preliminary interviews with clients to determine client’s appropriateness for the program to include: reviewing documents from Probation and/or Department of Children and Family Services (DCFS), Coordinated Entry System (CES), Los Angeles Homeless Services Authority (LAHSA), and other medical documentation to ensure proper care and process of the client and their records; prepare files for periodic review and make recommendations.
- Provide social work case management and navigation services designed to assist clients and their families obtain stability.
- Provide transportation to clients as needed.
- Collaborate with the Family Advocates during the discharge planning process to ensure continuity of care from Transitional Housing to Permanent Housing.
- Maintain a database and collect all intake process data. Conduct and provide monthly intake reports to Supportive Housing Program Director.
- Act as St. Anne’s Family Services, designated Tuberculosis Liaison for the Los Angeles Homeless Services Authority (LAHSA).
Qualifications
- Bachelor’s Degree in a Behavioral Science and a minimum two years of full-time experience working with transition-aged youth, or a Master’s Degree in a Behavioral Science or Human Service.
- Bilingual in Spanish/English is preferred.
- Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.
- Physical ability to perform Pro-ACT, administer first aid/CPR and operate an Agency motor vehicle.
