- Progressing initiatives to facilitate the implementation of recommendations from the Sustainable Health Review, such as the enhancement of the outpatient experience through reform targeted at improving access to specialist services
- Developing a contemporary South Metropolitan Clinical Services Plan in alignment to health service delivery with changing patterns of need, making the most effective use of available and future resources, driven by innovation, sustainability, and efficiency
- Collaborating with other SMHS directorates and services, such as the Emergency Access Response team and Innovation team, to identify and progress SMHS-wide improvement initiatives to reduce barriers to care and streamline the patient journey
Job Description
The Health Service Planner leads and facilitates the development and implementation of health service redesign, integrated service delivery models, evaluation and other projects in collaboration with health service providers and key stakeholders to meet the key strategic directions of SMHS and the broader health system.Currently, we have two (2) fixed term positions for the duration of 12 months each.Initiatives that the suitable applicants may be involved in include:
- Supporting Outpatient Reform
- Facilitating clinical service planning projects( or initiatives) across SMHS.
