Job Summary
We are looking for a Community Health Access Coordinator candidate who can perform a variety of community outreach, resource linkages, Oregon Health Plan (OHP) enrollment and navigation interviews, and health education duties. This position will focus on providing engagement and support services, particularly to income constrained and/or OHP-eligible community members. This position focuses on providing culturally and linguistically relevant information and supportive care access. Use county and regional data to develop and implement targeted community education, communication, and outreach strategies. Increase referral pathways with providers and services to facilitate access to care for Linn residents. Provide OHP enrollment and navigation services. Expand collaborative partnerships with service providers and agencies in support of health access. Support awareness Attend required team, community, and regional meetings. Responsible for data collection and reporting to the funder. May assist clients in both individual and group settings.
Minimum Qualifications
High school diploma or GED is required. Ability to work with minimal supervision. Two years’ experience working with communicable diseases is preferred or prior training and experience with high risk populations to reduce risk and prevent communicable diseases which demonstrates the ability to perform the work described. Spanish speaking preferred. Must have valid Oregon Driver’s License.
