About the role:
We have an exciting opportunity to work in a dynamic team leading our approach to the provision of quality care to clients of the TACP Service. This position assists in the delivery of therapy and support services for clients of the Transitional Aged Care Program. This position works with a large multidisciplinary team to provide slow stream rehabilitation to clients who have recently discharged from hospital. This role is supervised by Senior Allied Health staff within the Transitional Aged Care program to ensure high quality care. The tasks the Allied Health Assistant may undertake include: assisting clients with their Home Exercise Programs (HEP), escorting/community access, showering and bathing clients, and light domestic assistance. The incumbent will document all client responses to direct care given, including progress notes and clinical records in accordance with the plan of care and organisational procedures. This position is Monday- Friday, with the occasional weekend or public holiday required.TACP provides a package of services for up to twelve weeks duration to clients following discharge from hospital and who have potential to manage at home with rehabilitation and support services. The purpose of this intervention is to improve the client’s physical function, avoid further hospital presentations and to maintain their daily living skills. The Allied Health Assistant will support the service’s clients with personal care, transport, support rehabilitation goals and other activities as delegated by the multidisciplinary team.Diversity and Inclusion at St Vincent’s:
