About the role

The Clinical Nurse Specialist Grade 2 (CNS2) Renal coordinates and delivers high quality nursing care to patients utilising advanced practice nursing skills in assessment, clinical problem solving and care planning.

The role of the Renal Outreach CNS2 is multifaceted, and includes, but is not limited to:

  • The nursing management of patients with Chronic Kidney Disease (CKD stage 4-5/eGFR

20), including transplant work-up and dialysis access coordination. * Providing pre-dialysis treatment options education (HD, PD & RSC).

  • Peritoneal dialysis (PD) training and ongoing PD and home haemodialysis (HHD) management.
  • Providing on call support and ongoing education to patients and clinicians.
  • Follow up and support for patients who have received a kidney transplant.
