About the OPPORTUNITY
The Great Southern Population Health Directorate provides a range of community-based health services across the region. Services are provided by multidisciplinary teams including nursing, allied health, public health, Aboriginal health and health promotion staff.
The aim of Population Health is to prevent disease and injury, as well as maintain and improve the health status through prevention, promotion and intervention strategies and programs that empower the individual and community to make choices which lead to a better health outcome. As many of the factors which determine health outcomes lie outside of the health system, a guiding principle for all Population Health staff is to work in partnership with other organisations.
This is a rewarding and interesting role where you will work within the Community Health Team to:
- Provide competent nursing care for clients within the school community, promoting self-care and personal responsibility for health including assessing the needs of individuals, families and groups and referring to other services where appropriate.
- Act as an advocate for individuals, families, and communities in Public debate in order to protect and promote the health of the population.
- Plan, implement and evaluate nursing care, health promotion and education sessions within schools and also to individual families and groups according to need and service priorities, involving multidisciplinary teams where required.

This is a Permanent or Fixed Term appointment at various hours per fortnight, with the possibility of extension(s) and/or permanency.
About You
To be successful in this role, you will have:
- Demonstrated knowledge and understanding of primary health care principles and the social determinants of health.
- Demonstrated knowledge and understanding of multicultural issues including indigenous populations.
- Effective communication and interpersonal skills including well developed written, verbal and group facilitation skills.
- The ability to travel throughout the region on a regular basis.
