Job description: BEHAVIORAL HEALTH/MAT ACCESS NAVIGATOR: The Behavioral Health Access Navigator at OnPoint plays a crucial role in providing direct care and support to individuals within the community, as part of an integrated behavioral health team. This position is designed for someone who excels at building trusting relationships with clients, offering timely interventions, and ensuring individuals can access essential services. The Navigator will be responsible for leading the initial appointment post-intake, assessing client needs, goals, and treatment aspirations, and creating individualized care plans to address those needs. They will also prioritize referrals, ensuring smooth and effective connections with treatment teams to facilitate a seamless transition into ongoing care. With a focus on professionalism, empathy, and client advocacy, the Behavioral Health Access Navigator will be a key resource in helping clients navigate the behavioral health system. This is a grant-funded position, and continued employment is subject to the availability of funding.PAY RANGE/BENEFIT PACKAGE:
Salary: starting at $55,181.18 annually – placement above minimum salary is based on experience.OnPoint Benefits:

REQUIRED QUALIFICATIONS:
- Bachelor’s degree in social work, psychology, human services, or related field.
- One year experience working with adults diagnosed with an intellectual/developmental disability or mental health disorder.
PREFREERED QUALIFICATIONS:
- Knowledge of person-centered planning process.
- Experience working with an underserved population or individuals facing health disparities.
- Experience with case management, care coordination, and connecting clients with healthcare and community resources.
- Previous experience in community outreach or collaboration with local agencies.
- Electronic Medical Records (EMR) experience.
- Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders.
GENERAL RESPONSIBILITIES:
- Registering walk-in access intakes needing MAT services and completing their initial IPOS document.
- Engage in activities to support the coordination of care across healthcare providers, helping clients navigate and access appropriate services to address health disparities.
- Work with priority populations, including veterans and indigenous communities, to address the social determinants of health identified during the intake process. This includes facilitating referrals and connecting individuals with resources to address barriers like housing, employment, and behavioral health needs.
- Assist clients with case management referrals, linkages to services, and follow-up coordination to support individuals facing health disparities, homelessness, trauma, and other complex health challenges.
- Educate clients on how to use the patient health portal, empowering them to actively manage their health and access important information.
- Engage in outreach activities to connect with underserved populations, collaborating with local community agencies to improve access to care and address health inequities.
- Support individuals seeking OnPoint services through the person-centered planning process, ensuring that care plans are tailored to meet each client’s unique needs and preferences.
- Complete warm hand-offs and introductions to the assigned treatment team to ensure a seamless transition and ongoing support for clients.
- Perform additional tasks as needed to support the program and client care.
