About the role

Grafton Community Health has an exciting career oppotunity for a Clinical Nurse Consultant (CNC1) to join their Palliative Care team. Working with members of the palliative care nursing team across the inpatient and community settings, providing a consultative and liaison focussed service aimed at supporting GPs, Community Nurses and the Residential Aged Care Facilities in the assessment, care and management of patients with life limiting illnesses.

If you have:

  • Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1) and a minimum of 5 years’ full time equivalent post registration experience and at least 3 years’ full time equivalent experience in Palliative Care and postgraduate qualifications in Palliative Care or area related to the clinical specialty
  • Recent experience and knowledge and proven clinical expertise and leadership skills in assessment and management of palliative care patients, and an ability to work collaboratively within a multidisciplinary framework
  • Advanced communication and interpersonal skills (including the use of coaching) across a variety of mediums and proven ability to write for a variety of documents
  • Understanding of and commitment to the effective and meaningful engagement of clinicians in the decision-making process that impacts on positive patient outcomes
